Quality Improvement Plan
Based on the results of the needs assessment in Unit 4, select a quality improvement model to apply to your chosen scenario. Use tools associated with your chosen
model to:

· Summarize the gaps and need for change found during the needs assessment.
· Describe the process for improvement and managing change by addressing the elements of management (planning, organizing, directing, and controlling).
· Create a high-level outline of your conclusions and proposed quality improvement plan.
· Seek and incorporate feedback from key stakeholders regarding the results of the needs assessment and recommended next steps in your quality improvement
plan.
· Finally, synthesize the needs assessment, feedback from stakeholders into an executive summary designed to be presented to the team of people mostly likely
to work on the project.
Submission Requirements
· Written communication: Writing is free of errors that detract from the overall message. Writing is clear, precise, and scholarly. Concepts flow in a logical
order.
· Length of paper: Your plan must be 5–8 pages long, double-spaced, excluding the cover page and references.
· References: Include at least five scholarly sources to support your opinions and ideas with correct in-text citations, and a reference list.
· Formatting: Format your paper (including references) according to current APA style and formatting guidelines.
· Font: Times New Roman, 12 point.
